I've been using pcbcart.com. I have had no problems dealing with them,
even though they are in china. You can get an immediate quote on your
board from their web site. There will be an extra cost for air shipment,
which should be minimal for such a light job. Their NRE is very low.
I just plugged it in. $63.37 for 15-day lead time, 10 boards 1" by 2".
Here's another interesting possibility:
http://www.batchpcb.com/
They batch together small prototype runs. They charge $2.50 per
square inch, which turns out cheaper than pcbcart for your order.
I don't know what their typical lead time is.
